Lead Form Templates

Lead form templates let you create customizable web forms that capture lead information from potential customers. You design the form fields, preview the form, and share it via URL, QR code, or embed it directly on your website.

Before you start:

  • You need administrator access to create and manage lead form templates.
  • Configure your lead statuses in Lead Statuses before creating forms.

How to Access

Navigate to MarketingLead ManagementTemplates.

Create a New Form Template

  1. Click New Template on the Templates page.
  2. Enter a Template Name to identify this form internally.
  3. Click Save to create the template.

Result: The template is created and you are taken to the form designer.

Design Your Form with the Form Builder

The form designer provides a drag-and-drop interface for building your lead capture form. You can add, remove, and reorder fields to match the information you need from potential customers.

  1. In the form designer, drag fields from the available fields panel onto your form layout.
  2. For each field, set the Label (what the customer sees), Field Type (text, email, phone, dropdown, etc.), and whether the field is Required or optional.
  3. Reorder fields by dragging them up or down in the form layout.
  4. Optionally add product selection fields so leads can indicate which services they are interested in.
  5. Click Save to preserve your form design.

Preview Your Form

  1. Click the Preview button to see how the form will look to potential customers.
  2. Test the form by filling in sample data to verify all fields work correctly.
  3. Make adjustments as needed and save again.

Share Your Form

Once your form is ready, you can share it with potential customers in several ways:

  • Direct URL — copy the unique form link and share it via email, social media, or messaging. The URL follows the format of your company name and form path.
  • QR Code — generate a QR code that links to your form. Print it on business cards, flyers, or vehicle wraps so potential customers can scan and fill out the form instantly.
  • Website Embed — embed the form directly on your website using the provided embed code. The form renders inline within your web page.

Tip: Create different form templates for different services. An HVAC inquiry form might ask different questions than a plumbing emergency form.

What Happens When a Lead Submits

When a potential customer fills out and submits your form, a new lead is automatically created in FieldPromax with all the information they provided. The lead appears on the Leads list with an Open status, ready for your team to follow up. Form submissions can also include product selections if you configured product fields in the form.
